Rotator Cuff Injury: Causes, Symptoms and Physiotherapy Treatment in Burwood

Rotator cuff injuries are among the most common shoulder problems seen by physiotherapists in Australia. Whether you have a rotator cuff tear, tendinopathy, or impingement-related pain, physiotherapy at Burwood Physio can help you recover — often without surgery.

What Is the Rotator Cuff?

The rotator cuff is a group of four muscles and their tendons that surround the shoulder joint: supraspinatus, infraspinatus, teres minor, and subscapularis. Together, they stabilise the ball of your upper arm within the shoulder socket and allow you to raise and rotate your arm.

Types of Rotator Cuff Injury

Rotator cuff injuries range in severity and type:

  • Rotator cuff tendinopathy — degeneration or irritation of the tendon, usually from overuse
  • Rotator cuff tear (partial) — a partial thickness tear in one of the tendons
  • Rotator cuff tear (full thickness) — a complete rupture, which may require surgical review
  • Calcific tendinitis — calcium deposits within the tendon causing acute, severe pain
  • Impingement syndrome — compression of the tendon under the acromion during arm elevation

Common Causes of Rotator Cuff Injury

Rotator cuff injuries can result from both acute trauma and gradual degeneration:

  • Repetitive overhead movements in sport or work (swimming, tennis, painting, construction)
  • Heavy lifting with poor technique
  • A fall onto an outstretched hand
  • Age-related tendon degeneration (most tears occur in people over 40)
  • Poor posture and scapular control

Symptoms of Rotator Cuff Injury

Common signs include:

  • Pain at the front or side of the shoulder, often radiating into the upper arm
  • Pain when lifting the arm above shoulder height or reaching behind
  • Night pain, especially lying on the affected shoulder
  • Weakness when rotating the arm outward or lifting
  • A clicking or catching sensation with movement

How Physiotherapy Helps Rotator Cuff Injuries

The majority of rotator cuff injuries — including partial tears and tendinopathy — respond well to physiotherapy. At Burwood Physio, our treatment approach includes:

Initial Pain Management

In the acute phase, we focus on reducing pain and inflammation through manual therapy, joint mobilisation, and activity modification. We’ll identify positions and movements that aggravate your tendon and give you clear strategies to protect it while healing begins.

Shockwave Therapy

For chronic rotator cuff tendinopathy and calcific tendinitis, shockwave therapy is one of our most effective treatments. High-energy sound waves stimulate tendon repair and can dissolve calcium deposits. Multiple studies support its use for shoulder tendinopathy unresponsive to exercise alone.

Exercise Rehabilitation

Rotator cuff strengthening is central to recovery. Your physiotherapist will design a progressive loading programme targeting the specific muscles involved, building from isometric (static) exercises through to sport- or work-specific movements. Scapular stability exercises are also essential — a stable shoulder blade gives the rotator cuff the foundation it needs to function correctly.

Manual Therapy and Dry Needling

Dry needling can help release muscle tightness in the surrounding shoulder musculature, including the posterior capsule and upper trapezius, which are commonly restricted in rotator cuff presentations.

When Is Surgery Needed?

Full-thickness rotator cuff tears, particularly in younger, active patients, may require surgical repair. However, research consistently shows that physiotherapy produces outcomes equivalent to surgery for many partial and even full-thickness tears — especially in patients over 60. Your physiotherapist will assess your presentation and refer for specialist review if needed.

How Long Does Recovery Take?

Recovery time depends on injury severity:

  • Tendinopathy: 6–12 weeks with consistent physiotherapy
  • Partial tear: 8–16 weeks
  • Post-surgical: 3–6 months for most functional activities
